Post-TIPS Monitoring

Post-TIPS monitoring

What is TIPS?

TIPS (Transjugular Intrahepatic Portosystemic Shunt) is a procedure where a new connection is created between two blood vessels in your liver. This procedure is guided by x-ray and is performed by an interventional radiologist (a doctor who specializes in using imaging techniques to treat diseases). TIPS is typically done to relieve the high blood pressure in the veins of your liver, which can otherwise lead to dangerous complications.

Post-TIPS monitoring

After the Procedure

  • 1. Recovery:
    • You'll be monitored for bleeding in your hospital room. You will likely be asked to keep your head raised.
    • There's usually no pain after the procedure.
  • 2. Going Home:
    • Most patients go home the day after the procedure, once they feel well enough.
    • Expect to rest for 7-10 days as you recover and return to your normal activities.
  • 3. Follow-Up:
    • Your doctor will likely recommend an ultrasound soon after the procedure to ensure the stent is working properly.
    • Additional ultrasounds will be done in a few weeks and at follow-up visits.

Guidelines for Post-TIPS Care

To ensure the best outcomes after your TIPS procedure, follow these guidelines:

  • 1. Measure your abdominal girth regularly and weight to monitor for signs of fluid buildup (ascites).
  • 2. Strictly follow the drug and diet regimen as instructed by your healthcare provider.
  • 3. Sleep on your back or left side to help with blood flow and comfort.
  • 4. Ambulate as much as tolerated, but limit physical activity for the first few weeks to allow for healing.
  • 5. Monitor for signs of complications:
    • Low blood pressure, increased pulse rate (Bleeding)
    • Fever or chills (Infection)
    • Significant swelling in legs or abdomen, rapid weight gain, or shortness of breath (rarely heart failure)
    • Disorientation, confusion, mood or behavior changes, slurred speech (Hepatic Encephalopathy)
    • Jaundice/ yellowing of skin or eyes (Altered Liver Function)
    • Pain or bruising at the puncture site.
  • 6. Attend follow-up surveillance for shunt function at one, three, and six-month intervals, and then every six months.
  • 7. Regular blood tests: Have serum sodium and liver and kidney function tests (LFTs, RFTs, INR) every 1–3 months as recommended by your doctor.
  • 8. Avoid alcohol and smoking, as these can worsen liver function.
  • 9. Contact your healthcare provider immediately if you experience:
    • Severe/ new abdominal pain
    • Difficulty breathing
    • High fever (above 101°F or 38.3°C) or chills
    • Confusion or significant changes in sleep, consciousness or alertness
    • Yellowing of skin or eyes

Conclusion

TIPS is a highly effective, non-surgical procedure that improves blood flow, reduces pressure in the liver, and prevents complications of increased liver pressure like bleeding or fluid buildup. By following your doctor's instructions, attending follow-up visits, and staying vigilant for any signs of complications, you will maximize the benefits of this treatment and improve your overall health.
